$125 up to 2 hours
Plus $60/hour, prorated, for the amount of time over 2 hours.
This fee includes the in-office evaluation, phone or in-person collateral interview, phone contact with referral source, and written report, when the report is due 6 or more business days from the scheduled appointment time. |

If the report is due within 5 business days of the scheduled appointment, the fee is $180 up to 2 hours, plus $90/hour, prorated for the amount of time over 2 hours.
All alcohol and drug evaluation services must be paid by cash or money order only. The 2-hour fee must be paid at or before the beginning of the session. Any extra costs must be paid (cash or money order) before the written report is sent. |
