Obtain a professional and thorough alcohol and drug evaluation with an OASAS approved, Certified Alcohol and Substance Abuse provider at Trance-Formative Services, LLC. Attend a single session, and either bring someone along who knows you well, or provide a phone number of someone who can be reached while you are in session.

Expect to be treated respectfully, and know you will receive a fair diagnosis, recommendations, and referrals, from an experienced, professional evaluator who is both a Licensed Mental Health Counselor and a Certified Alcohol and Substance Abuse Counselor.
Department of Motor Vehicle evaluations are mailed within 1 business day of completing the evaluation. Other evaluations which require written reports can take a few days longer, but are done as efficiently and quickly as possible. Your referral source will be contacted by phone, in addition to receiving a written report.

Please note - only the referral source will receive a copy of the report. If you wish a copy, you must request it from the referral source. However, if you wish to discuss the evaluation outcome, or have any questions after viewing a copy from your referral souce, you can do so over the phone or in person.
